Job Summary
Implement residents' personal care plans focusing on their strengths, preferences, and routines while providing comfort and companionship. Deliver holistic care addressing physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. Complete documentation to meet reimbursement and regulatory requirements. Work onsite with an interdisciplinary clinical team to ensure resident needs are addressed throughout their continuum of care. Hourly pay range of $39.00 to $55.00 commensurate with experience.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
