Job Summary
Implement residents' personal care plans with a holistic approach that addresses physical, spiritual, emotional, and clinical needs while prioritizing their strengths, preferences, and routines. Complete documentation to meet reimbursement and regulatory requirements. Provide therapeutic services to older adults across multiple diagnostic groups, including urinary incontinence, vestibular rehab, home safety assessment, and movement disorders, within our award-winning campus where all clients reside on-site. Work as a per diem outpatient physical therapist supporting the interdisciplinary clinical team to ensure resident needs are addressed throughout their care continuum.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
