Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Collaborate with the interdisciplinary clinical team to address physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, and home safety assessment. Complete documentation to meet reimbursement and regulatory requirements for all clients living within the campus. Hourly pay range of $39.00 to $55.00 commensurate with experience; requires a valid state Physical Therapy license meeting Medicare and state requirements.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
