Job Summary
Conduct initial and periodic reassessments of participants' physical condition to develop personalized, evidence-based treatment plans including therapeutic exercise, gait training, and pain management. Record treatment encounters in the EMR system and develop, update, and implement Plans of Care with clear goals. Supervise program aides and collaborate with the interdisciplinary team to coordinate care, lead group exercise programs, and assist with discharge planning. Provide training to family members and advocates while actively participating in Quality Improvement processes. Work per diem in both clinic and community settings, adapting treatment plans to meet individual participant goals.
Required Qualifications
- Bachelor's or Master's degree in Physical Therapy
- 2 years' experience as a Physical Therapist
- Valid CPR certification or willingness to obtain
- Reliable transportation and a valid driver's license
- Position requires the ability to lift and move objects weighting up to 35 pounds without assistance
Desired Qualifications
- Experience in an outpatient, home health, or PACE setting preferred
