Physical Therapist Per Diem

A Physical Therapist administers physician-prescribed physical therapy to patients in their place of residence on an intermittent basis, under the direction/supervision of clinical leadership. Responsibilities include initial assessment, plan of care development and modification, reassessments every 30 days, improving residual disabilities, coordinating with home care personnel, and supervising personnel as required. Requires graduation from an accredited PT program (Bachelor’s/Master’s/Doctorate) and current state licensure; two years’ home health experience preferred; knowledge of Medicare OASIS documentation.
