Evaluate patient conditions, establish measurable treatment goals, and develop individualized therapy plans for home health patients. Provide effective care using approved techniques while documenting progress, updates, and treatment notes accurately. Supervise and coordinate care with PTAs and HHAs, educate families on exercises and adaptive equipment, and assess home environments for safety modifications. Ensure all services meet HIPAA standards and collaborate with nursing, social work, and other clinical staff. Requires an active FL PT license, minimum one year of experience, and current TB test and CPR certification.
