Job Summary
Initiate skilled physical therapy treatments per physician orders and maintain written evaluations, progress notes, and discharge summaries. Confer with health care staff to enhance rehabilitation, submit supply requisitions, and update individual plans of care in the Inter-disciplinary Kardex. Provide input for meetings and assume Director Physical Therapy duties during absences, including supervision of Physical Therapy Aides. Maintain statistical records and assist with in-service training programs as requested. Requires a valid NYS PT license, two years of experience preferred, and ability to direct others in a float role supporting diverse residents across departments.
Required Qualifications
- Graduation from an approved school of physical therapy
- State license to practice physical therapy
- Valid NYS PT license
- Ability to direct others
- Intermediate or more advanced level English language proficiency (read, write, speak, understand)
