Physical Therapist

Initiate skilled physical therapy treatments in accordance with physician's orders while maintaining required contact with the physician. Maintain written evaluations, progress notes, and discharge summaries for individuals in the physical therapy program. Confer with other health care staff regarding special needs to enhance rehabilitation or maintenance of function. Provide input for meetings and attend during absences of the Director Physical Therapy. Maintain statistical records of individuals' treatments and record updates to the Inter-disciplinary Kardex. Submit requisitions for supplies needed in treatment to the Director Physical Therapy. Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation. This role requires a state-specific Physical Therapy license, two years' experience preferred, and the ability to direct others. Full-time availability only at Elderwood at Lockport.
