Job Summary
Initiate skilled physical therapy treatments per physician orders while maintaining required contact and updating resident plans in the Inter-disciplinary Kardex. Maintain written evaluations, progress notes, and discharge summaries; confer with health care staff to enhance rehabilitation; and submit supply requisitions to the Director. Provide input for Resident Care Planning meetings and assume Director duties during absences, including supervision of Physical Therapy Aides and all necessary documentation. Full-time position at Elderwood at Waverly, a 4-star skilled nursing facility in Waverly, NY, offering a continuum of care with memory care and Adult Medical Day Services. Requires a state license, graduation from an approved school, and two years of experience preferred.
Required Qualifications
- Graduation from an approved school of physical therapy
- State license to practice physical therapy
- Ability to direct others
- Interest in physical therapy treatment programs for the geriatric and subacute populations
- Ability to read, write, speak, and understand the English language at an intermediate or more advanced level
Desired Qualifications
- Two years' experience in physical therapy
