Job Summary
Conduct evaluations, develop treatment plans, and deliver skilled physical therapy services to children in homes throughout Arizona. Monitor patient progress, maintain timely documentation, and educate caregivers while collaborating with physicians and interdisciplinary providers. Full-time W-2 benefits include medical, dental, vision, 401(k), PTO, and annual CEU reimbursement. Competitive pay ranges from $84 to $118 per visit/hour depending on employment type and location.
Required Qualifications
- Active Arizona PT or DPT license
- CPR certification
- AHCCCS ID
- Article 9 certification
- Dependable transportation
- Strong interpersonal skills
