Job Summary
Evaluate patients for mobility and functional limitations, establish measurable goals, and develop individualized plans of care. Respond to physician orders, progress patients toward discharge, and document evaluations, progress, and education activities thoroughly in the EPIC system. Demonstrate awareness of safety precautions and contraindications while overseeing clinical performance of PTAs and students. Participate in team conferences, coordinate with physicians and families, and prepare discharge recommendations including equipment and home health plans.
Required Qualifications
- Minimum Bachelor's of Science in Physical Therapy
- New York State license and registration or valid temporary license
- Ability to evaluate and provide treatment interventions to patients with diagnoses ranging from simple orthopedic conditions to more complex neurological conditions
- Ability to work in a fast-paced department
- Ability to accurately and completely document in a timely manner utilizing the electronic medical record system EPIC
- Excellent communication and customer service skills
