Physical Therapist - Part-Time

Assess patients to develop and initiate treatment plans based on evaluation results, reviewing physician referrals and medical records to determine required physical therapy interventions. Provide direct patient care according to established protocols, implement treatment programs, and document findings, progress, and instructions for patients and caregivers. Complete written plans of care, develop goals based on evaluation findings, and communicate effectively with patients, caregivers, physicians, and insurance companies. Ensure timely documentation of daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports. Responsible for patient care performed by assistants and rehabilitation technicians.
