Physical Therapist Part Time | Horizon Home Health & Hospice | Caldwell, ID

Perform comprehensive physical therapy evaluations in patient homes and develop individualized treatment plans focused on functional mobility, safety, and independence. Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development while conducting home-safety assessments and recommending appropriate DME or environmental modifications. Manage a field-based caseload with strong organizational efficiency and coordinate care with interdisciplinary team members to ensure seamless service delivery. Maintain accurate, timely documentation in accordance with regulatory and organizational standards and communicate effectively with physicians, caregivers, and internal team members. Support safe transitions from hospitals, SNFs, and outpatient settings.
