Physical Therapist, Part Time, Home Health

Conduct home visits to clients in designated territories to assist with recovery from injury, illness, surgery, or hospitalization. Perform diagnostic tests measuring mobility, range of motion, transfer status, strength, balance, and ambulation patterns. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises, gait training, and balance work. Continually assess and revise care plans while participating in multidisciplinary discussions with nursing. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires an active PT license with one year of clinical experience preferred; previous home health experience welcomed.
