Job Summary
Conduct physical therapy evaluations and carry out treatment plans for individuals with developmental disabilities in clinic, day hab, and school settings. Impart agency philosophy of independence and potential to children, families, and staff while collaborating with interdisciplinary teams and guardians. Ensure valid prescriptions are written, maintain confidentiality, and complete all paperwork in accordance with agency and Medicare/Medicaid regulations. Maintain treatment areas, manage department equipment, and attend team meetings as designated. Work part-time, 15 hours per week during the school year with evening availability as duties demand.
Required Qualifications
- Bachelor's degree in Applied Science
- Completion of accredited Physical Therapy program
- License or eligible for license as a Physical Therapist in New York State
- Current New York State registration as a Physical Therapist
- Valid New York State driver's license
- Must have demonstrated commitment to performing job duties and tasks at a high level of performance
- Maintains CPR and First Aid Certification
- Ability to pull and push wheelchairs up to 250 – 300 pounds
- Must be available for evening work
- Must remain in compliance with mandatory trainings per agency policy and per all agencies and governmental entities that are billed and govern the various clinics
