Physical Therapist - Part Time

Job Summary

Conduct physical therapy evaluation and treatment for PACE participants across acute, restorative, and maintenance levels, developing individualized treatment plans and educating families on exercise programs and durable medical equipment use. Perform home visits as needed, document activities and participant responses in charts, and contribute clinical assessments to the Interdisciplinary Team for whole-person care planning. Manage DME inventory, oversee group maintenance exercises, and maintain quality assurance, safety, and infection control standards while adhering to all regulatory documentation requirements.

Required Qualifications

  • Bachelor's Degree in physical therapy
  • Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by CMS
  • Active Physical Therapist license issued by California
  • A minimum of one year experience working with the frail or elderly
  • Current CPR/BLS certification
  • Ability to thrive in a fast-paced, evolving environment with comfort navigating ambiguity, adapting quickly, and contributing to continuous improvement
  • Strong learning and growth mindset, including seeking feedback, engaging in healthy debate and using data and curiosity to inform decisions
  • Acts with integrity and ownership, considering the broader organizational impact, doing the right thing, and following through to deliver results
  • A state issued driver's license, personal transportation, and auto insurance as required by law
  • Proof of vaccination or a signed declination form for COVID-19 and influenza
  • Compliance with applicable infection control protocols, personal protective equipment (PPE) requirements, and vaccination requirements

Desired Qualifications

  • 2+ years as a practicing PT
  • Doctorate of Physical Therapy (DPT)
  • Bilingual: Spanish/English strongly preferred; other languages a strong plus
  • PACE experience and/ or ADCH/CBAS experience
