Job Summary
Perform initial evaluations and develop individualized treatment plans for patients in their homes to improve mobility and regain independence following illness, injury, or surgery. Provide therapeutic exercises, manual therapy, and mobility training while assessing progress and adjusting care plans. Educate patients and caregivers on safety and exercises, maintain accurate documentation, and collaborate with interdisciplinary teams while adhering to home health and Medicare guidelines. Requires a current Physical Therapist license, OASIS knowledge, and experience with EMR systems such as Kinnser. This Level 2 role includes health, dental, and vision insurance, paid time off, and flexible scheduling.
Required Qualifications
- Current Physical Therapist license
- Graduate of accredited PT program
- Valid driver's license and reliable transportation
- Possesses and maintains current CPR certification
- Knowledge of OASIS
- This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment
Desired Qualifications
- 1–2 years of experience preferred in Home Health
- Strong communication skills
- Ability to work independently
- Experience with EMR systems such as Kinnser
