Job Summary
Assume responsibility for interdisciplinary assessment of hospice patients and provide physical therapy services according to a written physician's plan of care. Conduct daily evaluations of therapeutic and functional status, including muscle strength, mobility, gait, and range of motion, while assessing the home environment. Prepare clinical notes and progress summaries based on goal attainment, communicate with the physician and other practitioners regarding the hospice plan of care, and report patient reactions to treatment or condition changes. Identify patient and family/caregiver needs as necessary and provide in-service education programs for nursing organization personnel. Requires a valid driver's license and access to transportation for agency travel.
Required Qualifications
- Licensed to practice as physical therapist within the State, if applicable
- Passed an examination for physical therapists approved by the state
- Possesses a degree from a baccalaureate or master's program in physical therapy approved by the Commission on accreditation in Physical Therapy Education (CAPTE) or any successor organization of the CAPTE
- Two years of appropriate experience as a physical therapist
- Possesses and maintains current CPR certification
- Must have and maintain a valid driver's license
- Maintain automobile insurance coverage
- Have access to an automobile
