Job Summary
Provide outpatient physical therapy evaluation and treatment planning for patients with targeted goals, develop personalized treatment plans, and coordinate care within a multidisciplinary team. Responsibilities include educating patients and family caregivers on PT techniques, managing a caseload, and delivering patient-centered care across Oakdale and Sayville clinics. Opportunities include mentorship, leadership development, and residency pathways (OCS) with in-house support; a base salary range of $85,000-$105,000 plus incentive opportunities. Valid NY licensure or eligibility is required, along with First Aid, AED, and CPR certifications.
Required Qualifications
- Licensed Physical Therapist in NY or eligible
