Job Summary
Deliver one-on-one physical therapy to older adults in private residences, senior living communities, and memory care settings to improve mobility and functional independence. Document sessions effectively within the FOX EMR system and collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Incorporate telehealth and virtual care components to extend access and support patient continuity. Manage your own treatment schedule with autonomy while meeting physical demands for patient transfers and driving to client locations. Full-time and part-time roles available in New York, NY with H1B sponsorship.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Must be available for weekend shifts
- Must be able to lift 50 lbs
