Physical Therapist - Outpatient - Per Diem - $2,500 Sign on

Evaluate assigned patients, develop and modify treatment plans in accordance with physician direction, and manage delegation of therapy services. Collaborate with the interdisciplinary team to address discharge barriers, including equipment trialing, patient training, and home exercise program issuance. Effectively manage individual, concurrent, and group treatment sessions while ensuring clear communication of plans of care throughout the length of stay. Complete comprehensive evaluations, daily documentation, goal writing, FIM entries, and discharge documentation in compliance with state, federal, and intermediary guidelines. Maintain professional licensure in good standing, pursue continuing education, and participate in performance improvement initiatives to enhance service quality.
