Provide outpatient physical therapy services including evaluations, treatment planning, documentation, and patient education. Responsibilities include developing and implementing prescribed PT treatment plans, performing evaluations/re-evaluations, coordinating with physicians and other departments, communicating with insurers, ensuring HIPAA compliance, wound care as indicated, and maintaining timely progress documentation. Responsibilities also cover education for patients and families, supporting other staff, and traveling between Lebanon and Nashville metro locations as required, all while maintaining current PT licensure and BLS certification.
