Job Summary
Conduct comprehensive evaluations of patients across all age groups using age- and disability-appropriate assessments to establish baselines and develop individualized treatment plans. Administer therapy consistent with the treatment plan, continuously re-evaluate patients, monitor progress, and adjust plans to ensure optimal outcomes. Collaborate with therapists and healthcare professionals to provide integrated, multidisciplinary care, while supervising patient care activities of aides, assistants, attendants, and volunteers. Provide education to patients and families to support recovery and self-care, and recommend necessary physical barrier modifications or ergonomic adaptations. Maintain clear, accurate documentation of patient care and tailor communication to the specific needs of patients, families, and stakeholders. This temporary coverage supports Cardiac and Pulmonary Rehab services from July 2026 to January 2027.
Required Qualifications
- Current Physical Therapist license for applicable state required
- Physical Therapist Compact Privilege for applicable state required
- BLS for Provider certification from the American Heart Association required
- Able to render patient services and may supervise Licensed Physical Therapy Assistants (LPTAs), Therapy Aides and students
- Excellent organizational skills to facilitate program development
- Demonstrates competency, as appropriate to the ages of patients served, in the ability to obtain and interpret information in terms of the patient's needs
- Knowledge of normal growth and development
- Understanding of the range of the treatment needed
- Skilled in-home program development and family training
- Knowledge in appropriate selection and use of DME (wheelchairs, walkers, etc.)
- Computer skills
