Physical Therapist - Outpatient - Healthplex

Evaluate patients, develop treatment plans and goals, and implement programs to improve physical dysfunction and daily living independence. Continuously reassess treatment plans and educate patients, families, and others on home programs and preventive measures. Prepare written documentation and communicate with physicians and healthcare providers regarding patient care. Maintain departmental policies, quality assurance standards, and infection control protocols. Participate in educational programs, special project teams, and related duties as assigned. Work within a collaborative outpatient rehabilitation team treating patients of all ages and abilities.
