Job Summary
Plans and conducts segments of outpatient physical therapy treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in an interdisciplinary team approach; evaluates patients, develops individualized treatment plans, administers therapy interventions, documents findings, and supervises assigned assistants. Emphasizes quality documentation, communication with care teams, and adherence to vaccination and E-Verify requirements as part of Adventist Health Ukiah Valley's clinical operations.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in state of practice
- CPR or Basic Life Support (BLS) certification
