Physical Therapist - Outpatient

Perform initial evaluations, develop individualized plans of care, and provide direct therapeutic treatments for pediatric, adult, and geriatric patients. Complete patient evaluations within 48 hours, implement treatment plans with appropriate goal modifications, and provide comprehensive patient and family education. Collaborate effectively with the interdisciplinary team and families while maintaining accurate, timely documentation that adheres to department policies. Ensure compliance with department protocols for productivity logs, billing, and timecards. This role supports targeted rehabilitation programs at Methodist Outpatient Rehab, a 158-bed nonprofit hospital serving over 80,000 patients annually in Sacramento, California.
