Physical Therapist- Outpatient

Deliver evidence-based, patient-centered rehabilitation services to restore function, improve mobility, reduce pain, and maximize independence. Evaluate patients, develop individualized treatment plans, and provide therapeutic exercise, manual therapy, gait training, and functional rehabilitation. Monitor patient progress to modify treatment plans as needed, educate patients and caregivers on home exercise programs, and collaborate with physicians and interdisciplinary teams to optimize outcomes. Maintain accurate EMR documentation while complying with hospital and regulatory standards.
