Physical Therapist- Outpatient

Deliver evidence-based, patient-centered rehabilitation services to restore function, improve mobility, and reduce pain. Evaluate patients, develop individualized treatment plans, and provide therapeutic exercise, manual therapy, gait training, and functional rehabilitation. Monitor progress, modify plans as needed, and educate patients on home exercise programs while collaborating with physicians and interdisciplinary teams. Maintain accurate EMR documentation and comply with hospital standards.
