Physical Therapist - Outpatient CHT

Establishes treatment plans and provides skilled physical therapy evaluation and treatment for patients with physical impairments, considering discharge needs and functional status. Educates patients, families, and healthcare providers while collaborating on referrals and delegating interventions to assistants, techs, and students based on competency. Completes documentation in accordance with departmental policies and performs assessments, reassessments, and consultations. Offers 1:1 patient care in a multidisciplinary outpatient setting at Graham Medical Center with flexible scheduling between 7AM and 6:30PM, excluding weekends and holidays.
