Physical Therapist- Outpatient

Deliver evidence-based, patient-centered rehabilitation services to restore function, improve mobility, reduce pain, and maximize independence. Evaluate patients, develop individualized treatment plans, and provide therapeutic exercise, manual therapy, gait training, and functional rehabilitation. Monitor progress, modify plans as needed, educate patients on home programs, and collaborate with physicians to optimize outcomes. Maintain accurate EMR documentation while complying with hospital and regulatory standards.
