Physical Therapist- Outpatient

Deliver evidence-based, patient-centered rehabilitation services to restore function, improve mobility, reduce pain, and maximize independence. Evaluate patients, develop individualized treatment plans, and provide therapeutic exercise, manual therapy, gait training, and functional rehabilitation. Monitor patient progress to modify treatment plans as appropriate and educate patients and caregivers on home exercise programs. Collaborate with physicians and interdisciplinary care teams to optimize outcomes while maintaining accurate EMR documentation and complying with regulatory standards. Work in a hospital-based outpatient setting with therapist-driven scheduling and one-on-one patient care. New graduates and experienced PTs are welcome to apply for this full-time, day-shift role.
