Job Summary
Responsible for the daily coordination and delivery of physical therapy services, developing and implementing individualized treatment programs, evaluating patients, planning therapy using functional tasks, maintaining documentation (evaluation results, treatment plans, progress notes), and communicating with patients, families, physicians, and the interdisciplinary team to coordinate care. Emphasizes collaboration with rehabilitation, medical, and nursing care plans; promotes ongoing professional growth and adherence to CARE values. Roles include providing direct therapy in various settings, establishing rapport with patients and families, and contributing to a multidisciplinary team in an outpatient setting.
Required Qualifications
- Active Physical Therapist license in state of practice (Upon Hire)
- 1-3 years experience preferred (as a licensed physical therapist)
