Job Summary
Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations to establish and implement individualized treatment plans. Monitor patient outcomes, adjust plans based on response, and manage discharge by providing home programs, teaching families, and coordinating community referrals. Collaborate with stakeholders to optimize care, maintain accurate documentation and daily statistics, and adhere to billing guidelines and accreditation standards. Perform manual techniques, patient positioning, and equipment setup while following infection control measures and safety protocols. Manage assigned caseloads, assist with coverage needs, and participate in performance improvement activities to enhance quality of care.
Required Qualifications
- Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation
- Licensed Physical Therapist in the State upon which you work
- Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
- Must be able to lift up to 50 lbs. from floor to waist
- Must be able to lift up to 10 lbs. from waist to overhead
- Must be able to lift and carry up to 40 lbs. at waist height a reasonable distance
- Must be able to push/pull with 30 lbs. of force
- Must be able to perform a sliding transfer of 150 lbs. with a second person present
- Must have functional use of senses to allow for effective communication
- Must be able to wear protective clothing as needed
