Job Summary
Evaluate, treat, and implement care for patients in accordance with American Physical Therapy Association standards and Essentia Health policies. Develop individualized treatment plans, provide patient and caregiver education, and manage discharge planning for a full caseload. Handle all ancillary duties including documentation, billing, scheduling, and supervising support staff. Collaborate with multidisciplinary teams and referring physicians to ensure cohesive care. Work where patient need is highest, flexing to other departments as schedules allow. This 36-hour outpatient role includes a weekend hospital rotation every 10-12 weeks. Candidates must hold a current license, BCLS certification, and meet driving requirements.
Required Qualifications
- Bachelor's Degree, Master's Degree, or Doctorate Degrees from a physical therapy accredited program
- Current license in the state performing services
- Ada, MN: Must meet the driving requirements and criteria acceptable to Essentia Health's insurer
- Homecare Departments: Must meet the driving requirements and criteria acceptable to Essentia Health's insurer
- BLS certified or ability to become certified within 1 month from hire date
- Moose Lake: Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency
- Moose Lake: Must meet the driving requirements and criteria acceptable to Essentia Health's insurer
- Moose Lake: Proof of current motor vehicle insurance
- Hermantown Wellness Center: Basic Cardiac Life Support (BCLS)
- Must be able to flex to other departments when home department schedule allows
- Weekend (Saturday & Sunday) rotation at St Josephs Hospital in Brainerd once every 10-12 weeks
- No Call Obligation
