Job Summary
Physical Therapist provides rehabilitative PT services for home health patients as part of an interdisciplinary team. Responsibilities include assessing muscle strength, mobility, gait, ROM and transfer capabilities; developing and implementing plans of care; instructing patients, families and caregivers; initiating therapy programs; supervising the PTA; participating in quality assessment and performance improvement; and documenting progress notes. Education required: Bachelor’s degree or master’s in physical therapy; Ohio PT license; valid driver’s license and CPR certification; minimum 2 years of PT experience in community/home health preferred. Shift is PRN with pay per visit and weekend differentials. Locations: Lebanon, Ohio (USA).
Required Qualifications
- Bachelor’s degree or master’s in physical therapy
- Current license as a Physical Therapist from the State of Ohio
- Valid Ohio driver’s license and access to reliable transportation
- Current CPR certification
- Complete a minimum of twelve (12) hours of in-services per year
- Minimum 2 years of physical therapist experience with community / home health preferred
