Physical Therapist needed for Otterbein Staffing Agency Home Health in Lebanon, OH. Provide rehabilitative physical therapy services to home health patients per plan of care, including evaluating therapeutic/functional status, gait training, ambulation, transfers, and home exercise programs. Responsibilities include instructing patients, families, and caregivers; reporting changes to the physician; initiating therapy programs and guiding staff; supervising the Physical Therapist Assistant; participating in quality assessment/performance improvement; coordinating with the home care team; and preparing timely clinical notes compliant with state/federal guidelines. Education requirement: Bachelor’s or master’s in physical therapy with a current Ohio license; CPR; valid driver’s license; ability to drive to patient locations; and minimum 2 years of PT experience (community/home health preferred). Shift is PRN with pay per visit and weekend differentials. Location centers on Lebanon, Ohio, United States.
