Physical Therapist

Perform initial and ongoing systematic patient assessments, conduct individual therapy regimens, and monitor patient responses to modify treatment plans as needed. Set goals, develop treatment plans, and teach and counsel patients and families while promoting continuous quality improvement. Supervise Physical Therapy Assistants and students, participate in discharge planning including adaptive equipment education, and complete appropriate documentation per department policies. Collaborate cooperatively to identify and solve patient-specific and facility-wide needs.
