Job Summary
Develop and implement a progressive in-home rehabilitation program as a Physical Therapist; serve as case manager and supervise Home Health Aides (HHA) and Physical Therapy Assistants (PTA) when appropriate; evaluates assigned patients, admits those meeting agency criteria, plans and coordinates treatment, performs reassessments, and discharges patients when goals are met or home health requirements are not met; develop care plans for HHA and PTA, delegate duties, and supervise per agency policy; assumes responsibility for case management and makes appropriate interdisciplinary referrals; performs patient care duties and treatments based on medical condition and age; requires Master’s or Doctorate in Physical Therapy, NBCOT certification, Hawaii state license, BLS CPR, and two years of PT experience (preferably in home health); includes a $2,000 retention bonus and comprehensive benefits; OHANA PACIFIC HEALTH is an Equal Opportunity Employer and participates in E-Verify where required.
