Job Summary
Develop and implement a progressive home health rehabilitation program as a Physical Therapist (PT) in patients' homes, serving as case manager, supervising Home Health Aides and PT Assistants, evaluating patients, coordinating and implementing treatments, re-assessing progress, and discharging when goals are met; delegate and supervise care plans for HHA and PTA; ensure regulatory compliance and collaborate with interdisciplinary resources.
Required Qualifications
- Master’s Degree or Doctorate Degree in Physical Therapy
- NBCOT-accredited occupational therapy program and fieldwork
- 2-step TB clearance
- Current Hawaii state license to practice physical therapy
- Current BLS CPR certification
- Two years of experience as a physical therapist, preferably in home health or related setting
