Job Summary
Physical Therapist – in-home and senior-living care in Novato, CA. Provide one-on-one PT to older adults in private residences and senior living communities, including evaluations, individualized plans of care, and evidence-based treatment to improve mobility, independence, and aging-in-place. Documents in FOX EMR, collaborates with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), educates patients, caregivers, and staff, and may include telehealth/virtual care to extend access and flexibility. Roles available across in-home, senior-living, and virtual care settings with options for full-time, part-time, and PRN paths. Must hold a valid PT license in practice state(s) or eligibility; degree from accredited PT program. In California, standard base pay range for Full-Time: $80,000-$114,000 annually; opportunities to earn more. EMT/CPR as applicable. Duties include in-home and community-based care, documentation, and driving to patient homes as part of Geriatric House Calls model. Equal opportunity employer.
