Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities, including evaluations and development of individualized plans of care. Deliver evidence-based treatment to improve mobility and functional independence, document in the EMR, collaborate with an interdisciplinary team (PT/OT/SLP/Fitness Specialists), educate patients, caregivers, and staff to promote safety and aging in place, and offer telehealth/virtual care when appropriate to extend access and flexibility. Qualifications include a valid PT license in practice area and a degree from an accredited PT program; option for telehealth and in-home visits under a Geriatric House Calls model.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- CPR certification
- Basic computer and EMR proficiency
