Job Summary
Physical Therapist position at Montello Care Center in Montello, WI. Provides and administers physical therapy to patients and residents under physician orders and plan of care, with responsibility for initial patient examination, evaluation, plan of care, and maintaining patient records. On-site PRN role (2–3 days/week) with benefits and in-house therapy program. Requires graduation from an accredited PT program and state licensure; CPR preferred. Emphasis on delivering quality rehab services within outpatient/ALF/ILF/LTC settings, adherence to professional standards, and proficiency with EMR and payer documentation.
Required Qualifications
- Graduate of an accredited education course in physical therapy
- Licensed in the state in which they will be practicing
- CPR Certification preferred
- Must not be excluded or sanctioned from state or federal program
- Proficient administrative and communication within department and facility and company
- Thorough knowledge of principles, methods, materials, and equipment used in the practice of their discipline
- Thorough knowledge of all payer source documentation and payment requirements
- Ability to use analytical skills to develop outcome driven solutions
- Prior EMR/computer experience
