Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating and treating patients in their private residences or independent living facilities. Develop and deliver individualized physical therapy programs to restore function, instruct patients and caregivers on essential skills, and implement therapeutic techniques including exercise, manual therapy, and adaptive device training. Screen, examine, and evaluate patients to establish diagnoses, design treatment plans, and perform reexaminations to modify care as needed. Supervise Physical Therapy Assistants, students, and support personnel while ensuring adherence to documentation, safety protocols, and department schedules.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
