Job Summary
Evaluate and treat older adults in their private residences or independent living facilities, delivering personalized physical therapy programs to restore function and promote independence. Screen, examine, and assess patients to establish diagnoses, develop treatment goals, and implement therapeutic techniques including manual therapy, gait training, and adaptive device application. Instruct patients and caregivers on mobility skills, safety interventions, and chronic disease management while directing activities to maintain health. Supervise Physical Therapy Assistants and support personnel as needed, collaborate with the rehabilitation team to address comprehensive patient needs, and ensure adherence to clinical documentation and safety standards. Maintain equipment, order supplies, and perform home mobility assessments to recommend safe community reintegration.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
