Job Summary
Physical Therapist needed for Mobile Outpatient Therapy in-home outpatient rehabilitation for older adults. Will evaluate and treat patients in private residences or independent living facilities, develop and deliver PT programs, and coordinate with the rehabilitation team. Responsibilities include supervising assistants and students as needed, ensuring timely documentation and billing, collaborating with clinicians, adapting plans of care, promoting patient safety, and ordering/maintaining equipment. Role involves flexible schedule across home and community settings, with emphasis on functional restoration, caregiver education, falls prevention, and mobility training. Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master’s degree in Physical Therapy; or Master’s in a Physical Therapy related field with Bachelor’s in Physical Therapy; or Bachelor’s in Physical Therapy plus 5 years of PT experience
- Foreign degree evaluation required to determine US equivalence
- Must have a valid driver’s license or reliable transportation to patients’ residence
