Job Summary
Mobile in-home Physical Therapist providing outpatient rehabilitation for older adults in home and in independent living settings. Evaluates and treats patients in functional environments, develops individualized therapy plans, coordinates referrals to other disciplines, educates patients and caregivers, supervises PT staff when applicable, and conducts case management in home-based care. Flexible schedule in various home and community settings; opportunity for visa sponsorship (H-1B) and relocation support; in-home mobility, gait, balance, and activity-assistance services across the U.S.; 35 hours per week with a $5,000 bonus potential.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Master's degree in Physical Therapy; or Master’s in a Physical Therapy related field plus bachelor’s
- Bachelor’s degree in Physical Therapy with 5 years of progressively responsible PT experience (or equivalent)
- Must have valid driver’s license or reliable transportation
