Job Summary
Physical Therapist for mobile in-home outpatient rehabilitation in the Peoria, IL area. Provides evaluation, development, and delivery of PT programs in home and in independent living facilities, with focus on functional restoration, caregiver training, and safety. Responsible for scheduling, documentation, referrals to other disciplines, supervision of PTAs and students as applicable, and adherence to licensure requirements. Requires a PT license, Master’s degree (or equivalent), eligibility for licensure, and the ability to travel to patient residences; position offers visa sponsorship and relocation assistance. Includes a $5,000 bonus and a posted annual salary range.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice
- Master’s degree in Physical Therapy; OR Master’s in a PT-related field plus Bachelor’s in PT; OR Bachelor’s in PT plus 5 years of progressive PT experience
- Qualifying PT degree earned outside the U.S. must be evaluated as equivalent to U.S. PT degree
