Job Summary
Provide in-home outpatient physical therapy for older adults, delivering and adjusting individualized therapy programs to restore function and promote independence. The PT assesses patients’ needs, develops treatment plans, delivers therapeutic interventions (e.g., therapeutic exercise, gait/balance training, manual therapy, electrotherapy, and functional training), educates patients and caregivers, coordinates referrals with the rehabilitation team, supervises PTAs and students as applicable, maintains timely documentation, and supports safe home and community mobility. The role emphasizes flexible scheduling across home-based and community settings, with a focus on high-quality, compassionate care and adherence to licensure requirements. Eligibility includes an active PT license, Master’s degree (or equivalent), and ability to travel to patient residences; visa sponsorship and relocation assistance are provided.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master’s degree in Physical Therapy OR Master’s in Physical Therapy related field in addition to a Bachelor’s in Physical Therapy OR Bachelor’s in Physical Therapy plus 5 years of progressive PT experience
- Qualifying Physical Therapy degree earned outside the U.S. must be evaluated for U.S. equivalency
- Must have a valid driver’s license or reliable transportation to/from patients’ residence
