Job Summary
Mobile Outpatient Therapy Physical Therapist in-home outpatient rehabilitation for older adults, delivering therapy in patients' homes or in independent living facilities. Responsibilities include evaluating and treating patients, developing and implementing therapy programs, coordinating with the rehabilitation team, referring to other disciplines as needed, educating patients and caregivers, supervising PTAs and students as allowed, maintaining documentation and safety, and adapting plans of care to individual needs. Flexible schedule across various home and community settings with a focus on restoring independence and function.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master’s degree in Physical Therapy; or Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence
