Job Summary
Physical Therapist – Mobile Outpatient Therapy provides in-home outpatient rehabilitation for older adults, conducting assessments, developing and delivering individualized therapy programs, and educating patients and caregivers. The role includes supervision of PT assistants and support personnel, collaboration with the rehabilitation team, and ensuring timely documentation, safety, and adherence to treatment plans. Services cover therapeutic exercise, gait and locomotion training, manual therapy, neuromotor development, functional training, falls risk assessment, home and community mobility evaluations, and integration of health and wellness programming. Required Qualifications
- Valid Physical Therapy license in state(s) of practice or proof of license eligibility
- Master’s degree in Physical Therapy or Master’s in a related field with Bachelor’s in Physical Therapy or Bachelor’s in Physical Therapy plus 5 years progressive PT experience
- Must have valid driver’s license or reliable transportation
- Ability to work in in-home/outpatient settings
